ABSTRACT

ABSTRACT Purpose The predictors of trifecta achievement in partial nephrectomy (PN) were poorly inquired and remained a controversial area of discovery. To evaluate predictive factors of trifecta achievement in patients undergoing PN. Materials and Methods A systematic literature search was performed to identify relevant articles. Only studies focusing on postoperative trifecta achievement and exploring its predictor with multivariable analyses were included. The trifecta achievement was defined as negative surgical margins, warm ischemia time <25 minutes, and no complications. Merged odds ratio (OR) and 95% confidence interval (CI) were used to evaluate the predictive effect. Results Thirteen studies with 7066 patients meeting the inclusion criteria were included. The rate of trifecta achievement ranged from 43.3% to 78.6%. Merged results showed that preoperative eGFR (OR: 1.01, 95% CI: 1.00, 1.02, P=0.02), operative time (OR: 0.99, 95% CI: 0.99, 1.00, P=0.02), estimated blood loss (OR: 1.00, 95% CI: 1.00, 1.00, P <0.001), tumor size (OR: 0.70, 95% CI: 0.58, 0.84, P <0.001), medium (OR: 0.39, 95% CI: 0.18, 0.84, P=0.02) and high PADUA score (OR: 0.23, 95% CI: 0.08, 0.64, P=0.005) were independently associated with trifecta achievement. A publication bias was identified for tumor size. Sensitivity analysis confirmed the stability of result for tumor size. Conclusions Larger tumor size, medium and high PADUA score are associated with decreased probability of trifecta achievement. After verifying by further high-quality studies, these variables can be incorporated into tools to predict probability of trifecta achievement during clinical practice.

ABSTRACT

ABSTRACT With the continuous improvement of people's material living standards, public health and national fitness has become a social topic, and has been widely concerned by the public, in terms of the traditional physical education and impact of the concept of health. This not only needs the school to carry on the multi-directional adjustment to the physical education teaching, but also needs the physical education teacher to respond positively to this in order to adapt to the development needs of the times and society. This study uses the analytic hierarchy process (AHP) to construct the professionalization model of physical education teachers in Colleges and Universities, and makes decision on the index weight of the model through fuzzy comprehensive evaluation. The results show that the weight of the first-class indicators of P.E. Teachers' professionalization model from large to small is ability > quality > knowledge, and the consistency ratio (CR) of the model indicators is less than 0.1, and the fuzzy score of the model is 85 points. Therefore, the model of P.E. Teachers' professionalization constructed in this study has consistent indicators and shows good applicability. This does not only provide the basis for the professional development evaluation of College PE teachers, but also provides research ideas for the development of the sports industry from the perspective of public health.


RESUMO Com a melhoria contínua dos padrões de vida materiais das pessoas, a saúde pública e a saúde nacional tornaram-se um tema social, e tem sido amplamente discutido pelo público, em termos de educação física tradicional e impacto do conceito de saúde, que depende não só da escola para continuar o ajuste multidirecional para o ensino de educação física, mas também precisa do professor de educação física para dar uma resposta positiva a isso, a fim de se adaptar às necessidades de desenvolvimento dos tempos e da sociedade. Este estudo utiliza o processo hierárquico analítico (AHP) para construir o modelo de profissionalização dos professores de educação física em faculdades e universidades, e toma uma decisão sobre o peso do índice do modelo através de uma avaliação abrangente. Os resultados mostram que o peso dos indicadores de primeira classe do modelo de profissionalização dos professores de grande para pequeno é a capacidade > qualidade > conhecimento, e a razão de consistência dos indicadores do modelo é menor que 0.1, e a pontuação difusa do modelo é 85 pontos. Assim, o modelo de profissionalização dos professores de educação física, construído neste estudo, tem boa consistência de indicadores e mostra boa aplicabilidade. Isto não só proporciona a base para a avaliação do desenvolvimento profissional dos professores universitários de educação física, mas também fornece ideias de investigação para o desenvolvimento da indústria desportiva sob a perspectiva da saúde pública.


RESUMEN Con la mejora continua del nivel de vida material de las personas, la salud pública y la buena condición física nacional se han convertido en un tema social, y han sido motivo de una amplia preocupación en la población, con respecto a la educación física tradicional y su impacto en el concepto de salud. Esto no solo necesita que la escuela lleve a cabo el ajuste multidireccional de la enseñanza de educación física, sino también que el docente de educación física responda positivamente para adaptarse a las necesidades de desarrollo de los tiempos y la sociedad. Este estudio utiliza el proceso de jerarquía analítica (PJA) para construir el modelo de profesionalización de los docentes de educación física en los colegios y universidades, y analiza el peso del índice del modelo a través de una evaluación integral difusa. Los resultados muestran el peso de los indicadores de primera clase del modelo de profesionalización de los docentes, de mayor a menor, es capacidad> calidad> conocimiento, y el índice de consistencia (IC) de los indicadores del modelo es menor a 0.1, y la puntuación difusa del modelo es de 85 puntos. Por tanto, el modelo de profesionalización docente construido en este estudio tiene indicadores consistentes y muestra buena aplicabilidad. Esto no solo proporciona la base para la evaluación del desarrollo profesional de los profesores de educación física universitaria, sino que también ofrece ideas de investigación para el desarrollo de la industria del deporte desde la perspectiva de la salud pública.

@# Objective The structural equation model was used to analyze the influencing factors of mental health of family caregivers with schizophrenia, so as to provide scientific basis for targeted intervention measures. Methods On the basis of investigating the social dysfunction of schizophrenia patients and their family function, social support and mental health status, structural equation model was used to analyze the factors affecting mental health of caregivers and their effect indexes, the main fitting parameters included compare the fitted indices(CFI), goodness of fit index (GFI), the adjusted goodness of fit index(AGFI), normed fit index(NFI) and approximate error root mean square(RMSEA). Result The total score of psychological self-rating scale of carers was higher than that of normal people(all P<0.05). Only 19.0% of home care providers with schizophrenia felt no or little burden, 33.0% mild burden and 48.0% heavy burden. Structural equation model fitting index for: 2(58)=47.440, P=0.838, 2/df=0.818, the main fitting parameters GFI=0.968、AGFI=0.943, CFI=1.000, NFI=0.973, RMSEA=0.000, model fitting effect was good. The total effects of social support, family care, patient social function and care burden on the mental health of family caregivers were -0.016, -0.197, 0.114 and 0.118, respectively. Conclusions The family caregivers of schizophrenia have serious mental health problems, and the burden of care is heavy. improving social support, family care and social function of patients is an effective way to improve their care burden and mental health level.

ABSTRACT

Objective To explore the protective effect of gallic acid in Phyllanthus emblica on high glucose-induced apoptosis of pancreatic islet β cells, and to provide a reference for the discovery of natural compounds for the treatment of diabetes. Methods In vivo experimental model, wistar male rats were used as in vivo subjects and 50 mg/kg STZ was injected intraperitoneally. After the model was successfully established, 25 mg/kg of gallic acid was given orally, and the positive drug was Sitagliptin. After 4 weeks of administration, the blood was taken and the pancreas was removed for HE staining. Western blot was used to measure the expression of NLRP3 and TXNIP in pancreatic tissue in high sugar state. In vitro model, insulinoma cell line INS-1 cells were used as in vitro targets to establish high levels. In sugar-induced apoptosis model, INS-1 cells were cultured in glucose-free RPMI 1640 complete medium supplemented with 25 mmol/L glucose. Gallic acid was used as the test sample. Experiments were divided into normal controls, high-sugar models, and low, medium and high levels of gallic acid groups. The cell viability was measured by MTT assay. The mRNA expression of NLRP3 and TXNIP in INS-1 cells was detected by QPCR and Western blot, and the expression of NLRP3 and TXNIP protein was detected.Results (1) INS-1 cells were cultured in a medium with glucose concentration of 25mmol/L for 48h, and the apoptosis rate was increased compared with the control group (P<0.01), indicating that the apoptosis model was established successfully under high glucose conditions. (2) 10, 5, and 2.5 μmol/L GA were used to treat the control group and the high glucose model group cells respectively. The survival rate of the control group did not change significantly (P>0.05) . Compared with the control group, the expression of NLRP3 and TXNIP in INS-1 cells in the high glucose model group was significantly different (P<0.05);the protein expression level was significantly downregulated after GA treatment, and there was a statistical difference (P<0.05) . Compared with the control group, the expression of NLRP3 protein in INS-1 cells in the high glucose model group was statistically different (P<0.01), and the protein expression level was significantly downregulated after GA treatment (P<0.01) ; The protein expression level was up-regulated (P<0.05);the protein expression level after GA treatment was significantly down-regulated (P<0.05); (4) The expression of NLRP3 and TXNIP mRNA in INS-1 cells was increased in the high glucose model group compared with the control group (P<0.01) ; The expression of protein was significantly down-regulated after GA treatment (P<0.01) . Conclusion The cells were cultured for 48 h in glucose-free RPMI 1640 complete medium supplemented with 25 mmol/L glucose. GA has no effect on the proliferation of normal INS-1 cells. GA protects INS-1 cells from apoptosis under high glucose conditions. The mechanism may be related to GA down-regulation of NLRP3 and TXNIP gene expression.

ABSTRACT

Objective To study the inhibitory effect of ursolic acid on the proliferation of human papillary thyroid carcinoma cell line TPC-1 in vitro. Method TPC-1 cells were treated with different concentrations of ursolic acid (control group:0μM, experimental group:3μM , 6μM, 12μM);MTT assay was used to observe the effect of the growth of TPC-1 cells on different concentrations of ursolic acid at the same time;Apoptosis and cell cycle distribution of TPC-1 cells were treated with ursolic acid by flow cytometry;The expression of Bcl-2, Bax and Caspase-9 mRNA in TPC-1 cells were treated with ursolic acid by QRT-PCR;The expression of Bcl-2, Bax and Caspase-9 protein in TPC-1 cells were treated with ursolic acid by Western blot. Results MTT assay showed that ursolic acid inhibited the proliferation of TPC-1 cells in a concentration and time-dependent manner, and the IC50 at 24 h, 48 h and 72 h was 14.21 μM, 10.56 μM, 10.39 μM; Flow cytometry showed that ursolic acid inhibited the apoptosis of TPC-1 cells in a concentration-dependent manner, and the growth of TPC-1 cells was arrested in S phase;QRT-PCR showed that Bcl-2, Bax and Caspase-9 mRNA were expressed in the control and experimental groups, ursolic acid inhibited the expression of Bcl-2 mRNA in a concentration-dependent manner and up-regulated the expression of Bax and Caspase-9 mRNA;Western blot results showed that Bcl-2, Bax and Caspase-9 were expressed in the control and experimental groups, ursolic acid inhibited the expression of Bcl-2 protein in a concentration-dependent manner and up-regulated the expression of Bax protein and Caspase-9 protein. Conclusion Ursolic acid can significantly inhibit the proliferation and induce apoptosis of human papillary thyroid TPC-1 cells, providing some ideas for the treatment of thyroid cancer.

Objective@#To investigate the value of the content of neutrophil elastase (NE) in the expressed prostatic secretion (EPS) and seminal plasma (SP) as a combined predictor in the diagnosis of type IIIA prostatitis with secondary infertility.@*METHODS@#This study included 62 fathers (group A) and 67 infertile men (group B), all with type IIIA prostatitis, and another 57 controls with no genitourinary tract disease (group C). We measured the NE contents in the EPS and SP, obtained the results of routine semen and EPS examinations and Chronic Prostatitis Symptom Index (CPSI), and calculated the ratio of EPS NE/SP NE by binary logistic regression analysis.@*RESULTS@#The combined predictor of type IIIA prostatitis with secondary infertility was SP NE-2 × EPS NE. Among the 129 patients with type IIIA prostatitis, the combined predictor was correlated strongly negatively with the WBC count in the EPS (r = -0.914, P 0.05). The mean values of the combined predictor in groups A, B, and C were -2 238 (95% CI: -2 595 to -2 054), -1 511 (95% CI: -1 778 to -1 307), and -148 (95% CI: -181 to -118), respectively, with statistically significant differences between the cases and controls as well as between groups A and B (P <0.01). The area under the ROC curve of the combined predictor for the diagnosis of type IIIA prostatitis with secondary infertility was 0.71 (P <0.001).@*CONCLUSIONS@#The content of neutrophil elastase in the EPS combined with that in the seminal plasma contributes to the diagnosis of type IIIA prostatitis with secondary infertility, which is superior to either the neutrophil elastase content in the EPS or that in the seminal plasma used alone.

ABSTRACT

To find a credible nutritional screening tool for evaluating relationship between nutritional status and diseases in Chengdu female residents, the reliability and validity of a revised semi-quantitative food frequency questionnaire (SQFFQ) were tested. The validity was assessed by comparing the SQFFQ with the ‘standard’ method of 3 days’ dietary recall, and the reliability was assessed by comparing the first SQFFQ with the second SQFFQ at 4 weeks interval. Correlation analysis showed that, for reliability, the average correlation coefficient (CC) of 22 kinds of nutrients was 0.66 and reduced to 0.60 after adjusting for energy; the average of intra-class correlation coefficients (ICC) was 0.65. For validity, the average CC was 0.35 and remained stable after adjusting for CC of energy or nutrients. Validity of 17 nutrients in SQFFQ survey had correlation with result of 3 days’ dietary recall. The results showed that the revised SQFFQ can be used for investigating the role of nutrients in development of disease in Chengdu female residents.

<p><b>OBJECTIVE</b>To calculate the delay times and embedding dimensions of surface electromyogragh (sEMG) signal of the masseter and temporal muscle during clenching at intercuspal position (ICP), and to reconstruct the space phase of sEMG signal with method of nonlinear dynamics.</p><p><b>METHODS</b>Ten male and 10 female young volunteers with normal masticatory system were included in the study, the signals of surface sEMG of bilateral masseter and temporal muscles in the position of ICP were collected, and the delay times and embedding dimensions of the sEMG signals were calculated using C-C method, the space phase of the sEMG signals was reconstructed.</p><p><b>RESULTS</b>A program of nonlinear dynamic analysis of mastication muscles sEMG signals was developed base on the platform of Matlab. The signals of the sEMG of the masseters and temporals were analyzed with nonlinear dynamic methods, the delay times and embedding dimensions of each muscles sEMG signals were obtained, and the space phase of each sEMG signals were reconstructed. The graphs of the space phase indicated the typical characteristic of chaotic attractor.</p><p><b>CONCLUSIONS</b>The signals of the masseter and temporal muscles of surface sEMG during clenching at ICP of normal masticatory system show the typical characters of chaos, the methods of nonlinear dynamics are effective to analyze the sEMG signals of mastication system.</p>
